Refer when a client needs assessment or care beyond your training and role. Trainers can support suitable exercise, but should not diagnose disease or replace licensed health professionals.
When a Fitness Trainer Should Refer a Client to a Health Professional

Know the boundary early
The ACSM trainer outline describes exercise for healthy or cleared clients, screening, recognition of relevant symptoms and referral. Qualifications and local rules still matter. One certificate does not authorize every health profession.
Build a referral network and emergency plan before a problem. Know how to reach local emergency services and follow current first-aid/CPR training and facility procedures.
Urgent symptoms need urgent care
Stop exercise and seek urgent appropriate medical help for serious symptoms such as chest discomfort, fainting, severe or unusual breathlessness, or signs of an acute neurological emergency. This is not an exhaustive diagnostic list. Do not wait for a weekly check-in or keep testing symptoms under load.
The ACSM screening guidance explains why health and activity history matter before exercise assessments. Screening cannot guarantee emergencies never happen. An article is not emergency assessment.
When to arrange non-emergency referral
- Persistent, worsening or recurrent pain beyond ordinary short-lived training discomfort.
- Screening findings indicating a need for clearance or further advice.
- Rehabilitation after injury or surgery requiring a treating clinician or physiotherapist.
- Medical nutrition needs, suspected deficiency, or diets affected by disease or medication.
- Distress around eating, compulsive exercise or body image requiring qualified support.
Select an appropriate qualified professional. You do not need to diagnose a condition before referring.
Make the referral useful
- Describe observations or client reports without diagnosis.
- Explain why the issue is outside your role.
- Pause or modify affected work and follow professional restrictions.
- With permission, give a concise relevant activity summary.
- Resume when appropriate advice and your competence support it.
Do not alter medicines, promise treatment or sell a supplement as a workaround. Respect confidentiality. Safe coaching can continue within boundaries, but health needs come before completing the workout.
Common questions
Must I diagnose before referring?
No. Describe facts and refer through an appropriate process.
Can trainers change prescribed medicines?
No. That decision belongs with the appropriate prescribing professional.
Is referral a failure?
No. Recognizing limits is responsible practice.
